I have seen this happen twice while Supercharging my 2020 Model 3 DM LR. I suspect It could happen while home charging as well.

It has happened when charging in sub-freezing temperature weather. It wouldn't release when I select Unlock Charge Port to remove the charging port cable by the screen icon or using my phone. It happened once after going thru a brushless car wash, and once when driving in a snow storm then stopping to charge. I suspect water has gotten inside port and froze the mechanism.

The solution is to open the trunk and look on the upper left side where you will find a manual release cable for the port. You need to pull the release cable down, and at the same time reach around outside to remove the charging cable. This info can be found on Pages 182-183 of the Owners Manual.
